• Provides information to community resources, communicates with the patient's health plan and/or medical group, and coordinate benefits including Durable Medical Equipment, Home Caregivers, Pharmacy services, as well as other community resources

Sonnet Home Health clinical team is staffed with Registered Nurses, Licensed Vocational Nurses, Physical Therapists, Occupational Therapists, Speech Language Pathologists, Medical Clinical Social Workers.
